Appointment Policy

New patients must arrive 15 minutes prior to their scheduled appointment to allow time for chart preparation.

Established patients with changes to personal, medical, or insurance information must also arrive 15 minutes early to update their records.

Minor children (under 18 years old) must be accompanied by a parent/guardian at the first visit, or whenever there are questions regarding treatment. Minors not accompanied by a parent/guardian on follow-up visits must have an Unaccompanied Minor form, completed by a parent/guardian, on file and will be responsible for any co-payments or charges due at the time of service.

Certain types of appointments require a credit card number to hold an appointment time. This information must be provided at the time of scheduling.

Appointments that require a deposit must be paid with a credit card at the time the appointment is scheduled. Please contact the office for specific information about which appointments require deposits.

Cancellations

If you are unable to make your appointment, we require notice 24 hours prior for general dermatology appointments and 48 hours prior for cosmetic, elective, and surgical appointments.

If you need to cancel an appointment for which a deposit was required AND if we receive notice of your cancellation at least 24 hours prior to your scheduled appointment time, the deposit will be refunded or applied as a credit to your account.

Missed Appointments

There is a missed appointment fee of $25 for routine general dermatology appointments and $75 for cosmetic, surgical, or multiple appointment slot appointments.

Higher missed appointment fees apply for cosmetic appointments requiring a deposit or pre-payment.
